Diabetic Foot Ulcer

A diabetic foot ulcer [DFU] is a common complication of poorly controlled diabetes mellitus. In addition to poor control of blood sugar with treatment, diet, and exercise, factors that contribute to ulcer development include underlying neuropathy, peripheral vascular disease, and poor foot care. Without proper care, DFUs can easily become infected leading to osteomyelitis and amputation of lower extremities. Research has shown that the development of DFUs is preventable.

Amputation

The Corstrata Lower Extremity Amputation Prevention (LEAP) Program is founded on a clinical model that reflects best practices and is centered on current scientific evidence. 

  1. Reduce lower extremity amputations in individuals with diabetes 

  2. Identify and then stratify patients based upon their level of lower extremity amputation (LEA) risk 

  3. Tailor patient care and management strategies based upon individual LEA risk

  4. Educate health professionals caring for patients at risk for LEA and update  clinical practices to reflect current evidence

  5. Establish a standard educational program and self-management regimen for at risk patients to incorporate into their daily care 

  6. Identify and collaborate with local, community interdisciplinary resources that are integral to successful LEAP

Pressure Injury

A pressure injury (bedsore, pressure ulcer, decubitus ulcer) is an injury to the skin and underlying soft tissue caused by unrelieved pressure. When prolonged pressure is applied to the skin over a bony prominence, the blood vessels are compressed and blood flow is interrupted. Tissue death eventually occurs in the form of a pressure injury. Friction, moisture, and shearing (stretching or pulling) forces weaken the skin and also contribute to skin breakdown.
